Supporting your fertility and pregnancy care with simple referral options.

Referring a patient or self-referring?
 

Referrals are simple. Healthcare professionals and individuals can refer in a few minutes via form or email. All referrals are confidential and responded to within 1–2 business days.

For Healthcare Professionals

We welcome referrals from fertility specialists, GPs, endocrinologists and allied health professionals. Choose the method that suits your workflow:

  1. Download & email the referral form — complete digitally or by hand and email to youremail@example.com.

  2. Submit via online referral form — secure, mobile-friendly form. Upload notes if needed.

For Individuals or Couples (Self‑Referral)

No referral letter required. If you’d like support to optimise fertility or navigate early pregnancy, get in touch:

  1. Email your name, contact details and goals to info@hdfertilitynutrition.com.au

  2. Complete the self‑referral form and we’ll contact you to arrange a discovery call or consultation.

When to refer

Fertility & IVF

 

  • Unexplained infertility or unsuccessful IVF

  • Recurrent miscarriage or implantation failure

  • PCOS, endometriosis, thyroid or metabolic conditions

  • Weight management for conception (BMI <18.5 or >30)

  • Optimising egg & sperm health; supplement review

Pregnancy (Continuity of Care)

  • Nausea, constipation, or appetite changes

  • Gestational diabetes or high BMI

  • Vegetarian/vegan or restrictive diets

  • Nutrient adequacy (iron, folate, iodine, DHA)

  • Guidance for safe food practices in pregnancy
